Community Benefit Grants Program Request for Proposal (RFP)

Dear Nonprofit Community Partner:
Thank you for your interest in the Sutter Medical Center, Sacramento (SMCS) Community Benefit grants program. If you have previously partnered with SMCS to help us meet the needs of our Sacramento neighbors, thank you for your continued commitment. And if you are new to this grants process, welcome.
In 2011, Sutter Medical Center, Sacramento invested more than $2 million dollars locally through strategic community partnerships and grants. We will continue this tradition in 2012. For this portion of the Community Benefit program, SMCS will award grants up to $50,000 through a competitive process outlined in this RFP packet. It is anticipated that SMCS will award 10-20 grants.
In addition to grants awarded through this RFP process, the Community Benefit team will invite a small number of finalists to compete for a capacity-building grant valued at $50,000. All of SMCS’ investment is designed to strengthen the web of support to our most needy residents. And by strengthening our nonprofit sector to help deliver these valuable services, we believe we are helping to increase the quality of life in our region.
During this cycle of grant making, we will fund in these four categories:
- Homelessness: Transitional and permanent supportive housing; and programs in support of these placements.
- Maternal & Child Health: Emphasis on prenatal and early childhood prevention and intervention programs.
- Whole Health Stability: Prevention and intervention programs that address individual, family and/or community health issues and demonstrate health improvement.
- Connecting to a Primary Care Provider: Enrollment in benefit programs, as well as providing of primary care services.
